Winners VBHC Prize

2023

The MSKI-IPUs at UTHA are unique specialty condition-based models of care driving health care transformation at the practice and policy level. Opening in October 2017, the IPUs provide ‘360° whole person care’ for a diverse array of patients, including those from minorities and vulnerable, underserved communities, experiencing a range of musculoskeletal conditions. The IPUs are designed to manage physical limitations, alongside mental and social health concerns under a specialty condition-based payment model. Appropriate care is provided by highly coordinated multi-disciplinary teams, colocated within an outpatient facility, where providers are accountable for health-related outcomes (patient reported outcome measures, PROMs) and total costs for episodes of care. 2022

DHSV’s VBHC initiative aims to achieve value from a patient perspective by influencing the public dental system to shift away from a supply-driven health care system organised around volume, output and what clinicians prescribe, towards a patient-centered system organised around what patients need and value. The core value of the initiative is improving patient outcomes without raising costs, or lowering costs without compromising outcomes, or both. By examining and measuring variation in public dental sector, DHSV aims to reduce waste and re-invest savings to see more people who currently don’t access care. Read more

2021

Joinvasc is a public comprehensive stroke care program in Joinville, Brazil aimed at improving stroke prevention and outcomes (including PROMs). Joinvasc is a model where a team (comprised of medical staff, multidisciplinary health professionals, managers, and a patient association) implemented the best practices for stroke care and performed long-term data monitoring, achieving a substantial value improvement for patients and families in the whole stroke care pathway. Read more

Santeon Samen Beter programma (Santeon Together Better Program). The Santeon Samen Beter program is unique in which 7 hospitals closely work together on a daily basis for 14 medical conditions. The program involves over 90 multidisciplinary teams across the 7 hospitals. The aim is to improve health care in the Netherlands by reaching better patient outcomes at reduced costs for care provided by the 7 Santeon hospitals, and by sharing our learnings and results with other healthcare organizations.  Read more

Good Life with Osteoarthritis in Denmark (GLA:D®) is a not-for-profit initiative launched by a research team at the University of Southern Denmark in 2013 with the aim of implementing guidelines for the treatment of knee and hip OA in clinical practice in order to facilitate evidence-based, high-quality care of patients with OA. The NHN aims to continuously improve outcomes that matter most to cardiac patients. Subsequently, those patient relevant outcomes are delivered at the lowest costs. Since all relevant healthcare providers are involved in the NHN, patient centered pathways covering the full cycle of care are standardized and continuously optimized based on insights in outcomes.
